This may include communicating with healthcare providers involved in your care, managing billing and payment processes, and conducting quality assurance, compliance, and service improvement activities to ensure the highest standard of care.

Most uses and disclosures not described in this Notice require your written authorization. You may revoke any authorization in writing at any time.

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.

We reserve the right to change this Notice. Any revised Notice will apply to PHI we already have about you and any PHI we receive in the future.
